  • complete it using your computer and Adobe Reader version 11 (available for download free if you do not have it)

    • due date is January 15, 2014
    • download the sheet using the link below
    • complete the Excel spreadsheet form using Microsoft Excel (2003 version or later)
    • email the completed sheet (as an attachment) to Michelle Haggerty at the contact information below
    • please sign the actual document and mail or FAX the signed sheet to the address/FAX number below
    • you may, then, want to print it and/or save it for your records
